WMPC is a nonprofit organization that provides foster care and adoption services to children and families in Kent County, MI in partnership with Bethany Christian Services, Catholic Charities West Michigan, D.A. Blodgett—St. Johns, Samaritas, and Wellspring Lutheran Services. WMPC is piloting a performance-based funding model through a contract with the Michigan Department of Health and Human Services (MDHHS), utilizing a case rate that incentivizes permanency while allowing for flexibility and creativity in funds. WMPC utilizes software that will allow for predictive analytics to project successful outcomes, and flag cases that are at risk. WMPC seeks to empower communities to create better futures for children and families through innovation and collaboration.

HOW YOU WILL SPEND YOUR DAY
The Director of Care Coordination and Innovation is responsible for providing oversight of cases of children and their families receiving foster care and other services through WMPC and its partner agencies. This position directs the review and authorization of expenditures for placement and client support services to ensure the effective and efficient use of all resources and appropriate services to children and their families. The Director of Care Coordination and Innovation will be responsible for developing innovative strategies to serve children and families, including conceptualizing, developing, and facilitating strategies and plans for engaging community stakeholders.
•Remains knowledgeable and understands current treatment and service trends and promising and evidence- based practices ensuring equitable and trauma informed services for families and children.
•Remains knowledgeable of current research and evidence-based practice in foster parent recruitment and retention, family finding and kinship placements, child and youth behavioral and mental health, and other foster care related services and interventions and develops new strategies to serve children and families more effectively.
•Leads utilization management of placements and services to determine trends, systemic issues, cost data, and gaps in client care and service needs through research, case consultation, and program oversight.
•Serves as a trusted liaison with Kent County DHHS, MDHHS, Network180, Kent County 17th circuit court, education, health, our partners, and other systems impacting children and families serviced by WMPC.
•Collaborates closely with the Director of Performance and Quality Improvement to develop and implement strategies to improve the safety, permanency and wellbeing of children resulting in meeting and exceeding contractual performance measures.
•Motivates and leads a high-performing Care Coordination team.
•Responsible for ensuring timely and quality reporting to the Michigan Department of Health and Human Services (MDHHS) and Kent County DHHS, WMPC Board, the Council on Accreditation (COA), and other stakeholders.
•Ensures integration of programmatic initiatives with performance and quality improvement and finance teams.
•Cultivates relationships with other leaders at Community Based Care organizations as well as other professionals in the community to enhance programmatic work at WMPC.
WHAT WE NEED FROM YOU
Master’s degree and three years of progressively responsible experience required, including supervisory and/or
administrative leadership; clinical license preferred. Experience working for a child-placing agency preferred.
•Knowledge of child welfare principles/standards, practices, and systems (i.e. foster care, adoption, child placing agency licensing and placement procedures).
•Knowledge of historical racism, systemic discrimination, implicit bias, and the linkage to inequities in child and family outcomes.
•Understanding of youth-serving systems from the perspective of a youth, parent, or caregiver being served preferred.
•Knowledge and experience in trauma-informed practices.
•Interpersonal skills necessary to lead and interact with a diverse group of individuals and stakeholders.
